$199 "a month" buys a 28-day supply, and the page does not say whether refills are billed monthly or every 28 days (13 charges a year rather than 12). Compounded and not FDA-approved. Graded on the same rubric as every other provider on our scorecard.

The short version

Joi + Blokes is one company with two storefronts: Joi for women and Blokes for men. It is built around lab testing first, a health coach as your everyday contact, and a menu wider than any other hormone clinic we grade: testosterone, enclomiphene and bioidentical HRT, plus NAD+, sermorelin, rapamycin, glutathione and low-dose naltrexone, with lab panels you can buy on their own. Every product page prints its price before you start.

The prices come with strings. Subscriptions carry a three-month minimum, cancellation needs 30 days' notice, and nothing is refundable: not prescriptions, not subscriptions, not lab orders. The $99 a month advertised for testosterone is the 12-month plan, paid $1,188 up front; the shortest plan is $149 a month and still a three-month, $447 commitment.

On our rubric it earns Grade A, 13 out of 14, which is 13 points out of 14. It loses one point on the pharmacy: it names seven dispensing pharmacies, most with street addresses, but states no compounding standard for any of them. FDA issued Joi + Blokes a warning letter in June 2026 about its compounded weight-loss marketing; details are below.

What it is, and who it's for

Joi + Blokes was founded by Josh and Katy Whalen. Josh started Blokes after his own experience with low testosterone; Katy started Joi after struggling to get answers in perimenopause. The company says it practices telemedicine "across all 50 states," names its medical director and nurse practitioners, and pairs every patient with a health coach. Its coaches, it notes, "are not doctors and cannot prescribe."

The model runs in three steps, in its own words: "Complete your health history and order a lab test," "go over your results with a licensed health care provider," then "start your personalized treatment plan." Hormone therapy and rapamycin require that lab-and-visit route. NAD+, sermorelin and glutathione can start from a health form alone.

It suits a reader who wants several longevity therapies from one place, with labs to go with them, and who is comfortable committing for three months at a time. What it actually costs

Every product page shows two prices: a one-time purchase and a cheaper "Subscribe + Save" price, which Joi + Blokes describes as 20% off with free shipping. The "/mo" figure on the shop listings is the subscription price.

Subscriptions carry a three-month minimum. The one-time price is the way to try one supply.

Longevity line. NAD+ nasal spray is $150 for a one-month supply on subscription, or $187 one-time. NAD+ injections are sold as a 10-week supply: $400 on subscription (about $172 a month) or $500 one-time (about $215 a month). Sermorelin, as injections or troches, is a 12-week supply: $559 on subscription (about $186 a month) or $699 one-time (about $233 a month); the shop listing shows "$199/mo" for the same product, so go by the product page. Rapamycin, prescribed as sirolimus, is $83 a month, sold as $249 for three months, and needs labs first. Glutathione is $69 a month on subscription, VIP peptide $159 and LDN $63.

Testosterone. The TRT page says the subscription "starts from just $99 and includes all medications, clinician consults and follow-up labs." Its FAQ shows the full ladder:

  • Flex plan: $149 a month, three-month commitment, $447 up front
  • Commit & Save: $129 a month, six-month commitment, $774 up front
  • Full Year: $99 a month, twelve-month commitment, $1,188 up front

The plan includes testosterone cypionate, clinician visits, quarterly labs, coaching and unlimited messaging, and hair-loss or ED medication can be added for $1 a month each. The page's start button reads "$75" and does not say what that buys; ask before you pay it. Enclomiphene follows the same ladder and is billed quarterly.

Women's HRT works differently: $50 a month, billed quarterly, covers labs and clinician visits, and each hormone is billed on top, from $49 a month for estrogen capsules to $129 for the estrogen patch. Estrogen and progesterone can instead go to your local pharmacy, where insurance may help.

Labs and consults. Panels run from $149 (56 hormone markers) to $399 (71 markers) and $699 (110 markers, with a 60-minute clinician consultation). A 15-minute call with a health coach is free; a clinician consultation to review outside labs is $150 for 30 minutes or $225 for 60.

Signing up, refunds and canceling

If a clinician decides a prescription is not right for you, Joi + Blokes says "you will be immediately refunded." After that, its refund page is four sentences long, and they all say the same thing:

  • Prescriptions: "We cannot accept returns of prescription products. All sales are final."
  • Hormone subscriptions: you can cancel them, but "The subscription cannot be refunded, all sales are final."
  • Labs: "All diagnostic lab sales and lab subscription sales are final and cannot be refunded."

The Terms add the commitment. "By enrolling in subscription services, you agree to pay for a minimum of three consecutive months of service." After that, you may cancel "thirty days before the next monthly processing date," by emailing the company. The HRT page says the same in plainer words: "Cancel anytime after the first 3 months."

So the real cost of trying NAD+ nasal spray on subscription is three months, $450, not $150. The one-time price, $187, is the honest way to try a single month. Joi + Blokes does not bill insurance; HSA and FSA cards work through a checkout partner. Disputes go to arbitration under its Terms.

  • Testosterone cannot ship to 16 states, including Pennsylvania, Georgia and North Carolina.
  • To cancel, give 30 days' notice before the next charge.

Who prescribes, and who fills it

Prescriptions come from licensed clinicians, "either asynchronously or synchronously." The company names its medical director, a head nurse practitioner and an asynchronous lead nurse practitioner, and says hormone patients see a clinician at about six weeks and then every three months.

On the pharmacy, Joi + Blokes publishes more than most. Its Terms name the dispensing pharmacies outright: "Strive Pharmacy, Pharmacy Hub, Tailor Made Compounding Pharmacy, Cadre Labs LLC, Empower Pharmacy, LillyDirect Self Pay Pharmacy Solutions d/b/a GiftHealth, Inc., and Partell Pharmacy," most with street addresses. What it never says is whether any of them compounds under 503A or 503B rules, which is the one point it drops on our rubric. Product pages state that "the FDA does not verify the safety or effectiveness of compounded drugs," which matches FDA's own position1.

The FDA warning letter

On June 8, 2026, FDA's Center for Drug Evaluation and Research issued a warning letter to Joi and Blokes dba Joi + Blokes (MARCS-CMS 728283)2. That entity name is copied from the letter's recipient block, and it is the citation: search it in the [FDA's warning-letter database](https://www.fda.gov/inspections-compliance-enforcement-and-criminal-investigations/compliance-actions-and-activities/warning-letters) and the record comes up. The letter concerns the company's compounded semaglutide and tirzepatide weight-loss products, not the NAD+, peptide or hormone products graded here. We link the FDA's database rather than characterize what it alleges.

What the medicine can and can't do

Parts of Joi + Blokes' product copy go further than the evidence. Its sermorelin FAQ says that by month six "studies suggest potential 5-10% body fat reduction (without diet or exercise), a 10% increase in lean muscle mass." Its rapamycin page lists "Promotes a longer, healthier lifespan," which has not been shown in people. Where it's weak, and who should skip it

  • Skip subscriptions for a first try. Three months is the minimum, and nothing is refundable. Pay the one-time price for a single month.
  • Read the $99 for what it is. It is the 12-month testosterone plan, paid up front.
  • Check your state for testosterone. It cannot be shipped to 16 states, including Pennsylvania, North Carolina, Georgia and Minnesota.
  • Ask which pharmacy fills your order. Seven are named, with no compounding standard stated.
  • Discount the boldest claims on sermorelin and rapamycin.

The verdict, against the rubric

Joi + Blokes takes all four for price transparency, because every product page prints its price, one-time and subscription, before you start, and its commitment terms are published. It takes all four for its longevity line, the broadest among the hormone clinics we grade. It takes two for clinical oversight, with clinician consults inside the treatment price, one for pharmacy, for naming its pharmacies without a standard, and two for human support, with unlimited messaging and a dedicated coach. Thirteen out of fourteen: an A.

The A grades what Joi + Blokes publishes, and it publishes a lot. The commitment and the no-refund rule are the price of that breadth: buy one-time first, subscribe once you know a therapy suits you.
